CFM-CPH.06.10 Working at height unwanted worker or machine operator action error
Worker or Operator does not act as expected when working at heights, caused by:

fails to wear safety harness
fails to clip on, or clip on correctly
operator incorrectly operates equipment and strikes object
operator falls off ladder or elevated position
operator actions (pushing, pulling, using tools, etc.) causes them to lose balance at height
not identifying weakened: structure; ladder foot or head position, or; inappropriate or weakened anchor point – which can fail when loaded
