CFM-CRA.10.01 Supervisor does not understand requirements of tasks (issues unsafe instructions or approves inadequate work plan)
Supervisors issue instructions that compromise safety of workers and effectiveness of the works due to:

• no understanding of task requirements and hazards and required controls
• not undertaking or encouraging and reviewing, formal analysis of works (JSA, Permit, etc.)
• not confirming permits are issued by an authorised person after inspection of the worksite
• incomplete or inadequate permits prepared and approved for use
• unclear communication of requirements from supervisor to workers, e.g. not identifying actual task steps required, not requiring formal permits, etc.
• lack of formal process of training, assessment and appointment of supervisors
• requirements for required controls are known, but not reinforced or issued as instructions to workers
• conflicting goals (and instructions) – preferencing speed of activity over safety and sustainability
• not following site requirements for allocating and monitoring works
