CFM-CPA.03 Worker not competent, authorised or skilled
The training management, selection and/or authorisation process fails to provide skilled workers (and/or supervisors), caused by failing to:

confirm competencies /authorisations
impart sufficient knowledge that could be retained by the worker after being trained (worker suffers information overload)
conduct refreshers/ assessments
check skills are appropriate and up to date
confirm the information needed for the tasks is provided to workers/trainees in a timely fashion
confirm that supervisors and permit authorisers have sufficient skills to understand related hazards and required controls
